What You'll Do:
- Assess and monitor patients' physical and psychosocial status, intervening proactively to manage potential crises.
- Develop and implement individualized care plans in collaboration with patients, families, and the interdisciplinary team.
- Communicate effectively with the multidisciplinary team to ensure comprehensive patient care and successful discharge planning.
- Provide health education tailored to patients and families, integrating best practices in nursing.
- Maintain accurate documentation of care in the electronic medical record and ensure a supportive environment for all patients.
What You'll Bring:
- A degree from an accredited nursing school (BSN preferred) and a current NH Registered Nurse license.
- Proficiency in patient assessment, technical skills, and electronic clinical documentation.
- Strong communication and collaboration skills to foster teamwork and positive relationships within the healthcare setting.
- A proactive attitude towards professional development and a passion for improving patient outcomes.
