Who You Are:
As a Social Work-Case Manager for Catholic Medical Center (CMC), your primary responsibility is to identify and evaluate the social complexity of patients and families. In collaboration with the RN-Case Manager, you will conduct a thorough 360 evaluation of patients' needs, arranging, coordinating, monitoring, evaluating, and advocating for a comprehensive package of services to meet their specific and complex needs. Your role involves ensuring the provision of appropriate services and community resources, supporting individuals and families in managing various life issues causing difficulty or distress.
What You'll Do:
Under the direct supervision of the Director of Case Management and the Social Worker Lead, you will perform the following functions:
• Arrange, coordinate, monitor, evaluate, and advocate for a package of services to meet specific patient complex needs.
• Collaborate with the primary nurse, RN Case manager, and multidisciplinary team in addressing ongoing clinical and psychosocial needs of the patient.
• Assist in identifying patients in need of complex care coordination, preventing delays in discharge, and ensuring timely meeting of patients' needs.
• Monitor patient progress to ensure appropriate and timely care.
• Serve as a resource for day-to-day clinically based problem resolution and participate in daily unit clinical/discharge planning rounds.
• Ensure compliance with the advanced directive process and coordinate and facilitate family meetings.
• Respect the dignity and confidentiality of patients and uphold the team-oriented approach to Case Management.
• Collaborate with the interdisciplinary team to review, identify, and coordinate all aspects of care on assigned cases for a systems approach to healthcare.
• Interpret federal and state regulations, conduct quality management activities, and participate in the development or revision of hospital policies/procedures.
What You'll Bring:
• A Bachelor's Degree in Psychology, Sociology, or Social Work is required, with an MSW being highly preferred.
• Bring a minimum of two years of experience in case management and/or utilization management, or relevant professional experience. Additionally, three years of clinical experience is required, showcasing your expertise in the field.
• While licensure is not mandatory, it is preferred. We encourage you to obtain certification within the first three years of employment, demonstrating your commitment to continuous professional development.
