The HCH Nurse - MAT also participates in the care of all HCH patients by addressing their complex medical and behavioral health needs. The nursing process is used to conduct in-depth assessments to identify co-occurring medical, mental health and substance use disorders. As part of an interdisciplinary team, the HCH Nurse - MAT promotes a therapeutic environment for all patients.
What You'll Do:
- In collaboration with Behavioral Health Provider, performs initial assessments including medical, mental health, medication history, substance use history and social history.
- Communicates and meets with MAT treatment team and other practice team members on a regular basis, employing a harm reduction approach to patient care.
- Provides education to patients and the community regarding access to MAT treatment programs.
- Assesses and closely monitors patients through the induction, stabilization and maintenance phase of treatment.
- Provides management of scheduled nursing visits, urine toxicology screens, initial and ongoing lab testing, patient education and medication refills, as ordered by protocol or medical provider.
- Follows State and Federal guidelines in providing care to patients with substance use disorders, in collaboration with licensed prescribing physicians, APRNs, PAs, and pharmacists.
- Maintains accurate record keeping of all patient interactions in the electronic medical record (EMR); tracks prescriptions, urine testing, behavioral health provider appointments and PCP appointments.
- Works with prescriber to obtain prior authorizations as needed.
- Maintains availability to discuss any issues by telephone from patients related to side effects or any concerns about prescription. Document all calls in EMR. Consults with prescriber as needed.
- Reinforces prescriber's plan of care. Promotes continuity of care.
- With patient's consent, responsible for coordination of care with staff at other treatment facilities, i.e. ER, Mental Health Center, and other community based treatment programs.
- Coordinates with all HCH staff to promote adherence to patient confidentiality and right to privacy laws and regulations.
- Engages in continuing professional education, with a focus on substance use disorders and behavioral health care; maintains compliance with state and specialty certification continuing education guidelines.
- Participates in the ongoing evaluation and development of HCH policy and protocols related to the MAT Program.
- Maintains ongoing list of community resources.
- Performs triage and rapid clinical assessments for patients who present at HCH shelter-based clinics, in partnership with the HCH behavioral health team.
- Initiates appropriate emergency response in life-threatening or unusual situations in order to stabilize the patient; promptly and directly notifies HCH provider.
- Assesses and records patients' vital signs, physical signs and symptoms, and presenting complaints in preparation for clinic visit.
- Performs comprehensive bio-psycho-social assessment and conducts mental health screening and substance use disorder screening using evidenced based tools.
- Develops an individualized care plan based upon nursing assessment, in collaboration with interdisciplinary team as indicated. Provides health education and counseling to patient in support of self - care and health management goals.
- Refers patient to appropriate level and category of care on multidisciplinary HCH team, based upon assessment; communicates assessment and screening outcomes to medical and behavioral health providers.
- Ensures that all clinical assessments, diagnostic results and consult reports are up to date in the electronic medical record (EMR) in preparation for clinic visits by medical and behavioral health providers.
- Coordinates with health insurance carriers to obtain prior authorizations for procedures, diagnostics and medications, as indicated.
- Coordinates discharge plan for each clinic visit; arranges referrals for diagnostic and specialty care, with support for transportation, escort, medication assistance and translation as needed.
- Provides and documents face to face nursing care visits and patient encounters, as indicated for health education and chronic disease management; provides health education to individual patients and to patients in group settings in a culturally competent manner.
- Performs nursing care interventions as ordered by provider and in support of the provider for medication administration, point of care testing, and diagnostic procedures during each clinic session.
- Assesses patient safety in the context of shelter, transitional housing, or home setting; intervenes as indicated.
- Participates in Street Medicine, in collaboration with other members of the HCH staff, as schedule permits, for the purpose of case-finding, engaging and enrolling people experiencing homelessness into HCH care.
- Participates in ongoing program evaluation, strategic planning, and Quality Improvement (QI); tracks and documents in the EMR with integrity and accuracy; implements corrective actions as indicated, in support of QI initiatives.
- Assists with orientation of nurses newly employed by HCH team; serve as preceptor to nursing students placed at HCH clinic.
What You'll Bring:
- Graduate from an approved school of nursing.
- BSN preferred.
- A minimum of 3 years of professional nursing experience.
- Experience working with adults with substance use disorders, with case management and psychiatric experience preferred.
- Interest in working with vulnerable populations, with community health experience preferred.
- Licensed in State of NH
- Basic Life Support (BLS) certification required.
- Basic computer keyboarding and data entry skills required.
