What You'll Do:
- Perform triage and rapid clinical assessments for patients presenting at HCH shelter-based clinics, in partnership with the HCH behavioral health team.
- Initiate emergency responses in life-threatening or unusual situations to stabilize the patient and notify the HCH provider.
- Assess and record patients' vital signs, symptoms, and complaints in preparation for clinic visits.
- Conduct comprehensive bio-psycho-social assessments, including mental health and substance use disorder screenings using evidence-based tools.
- Develop individualized care plans based on nursing assessments, in collaboration with the interdisciplinary team; communicate outcomes to medical and behavioral health providers.
- Provide health education and counseling to patients in support of self-care and health management goals.
- Monitor patients' health status, learning needs, and ability to manage chronic illness while addressing homelessness.
- Provide face-to-face nursing care visits for health education and chronic disease management, in individual or group settings, culturally competent.
- Carry out nursing interventions as ordered by the provider, including medication administration, point-of-care testing, and diagnostic procedures.
- Assess patient safety within shelter, transitional housing, or home settings and intervene as necessary.
- Participate in Street Medicine to engage and enroll homeless individuals into HCH care.
- Monitor and manage patients through the MAT treatment phases, including induction, stabilization, and maintenance.
- Provide nursing management for scheduled visits, urine toxicology screens, lab testing, patient education, and medication refills.
- Maintain accurate documentation of all patient interactions in the EMR, including prescriptions, urine testing, appointments, and other necessary follow-ups.
- Work with prescribers to obtain prior authorizations as needed and provide consultation regarding side effects or concerns from patients.
What You'll Bring:
- Graduate from an approved school of nursing.
- BSN preferred.
- A minimum of 3 years of professional nursing experience
- Experience working with adults with substance use disorders, with case management and psychiatric experience preferred.
- Interest in working with vulnerable populations, with community health experience preferred.
- Licensed in State of NH
- Basic Life Support (BLS) certification required.
