Who You Are:
Catholic Medical Center is seeking an experienced and dedicated professional with a background in diabetes patient care. You will have experience as a registered nurse, preferably with a focus on medical/surgical nursing and diabetes education, or as a registered dietitian with expertise in clinical dietetics and diabetes care.
What You'll Do:
- Utilize clinical expertise to provide self-management education through classes and individual counseling sessions for patients and families/significant others.
- Collaborate with the Clinical Manager of DRI and other providers (MD, RN, RD, ARNP, etc.) to deliver ongoing diabetes self-management education.
- Educate hospital staff, physicians, physician assistants, and nurse practitioners on improving blood glucose control in hospitalized patients through intensive management.
- Provide education to hospital staff and prescribers on safety issues related to diabetes management and discharge planning.
- Evaluate the effectiveness of educational experiences through verbal feedback, return demonstrations, outcome measures, and patient/family satisfaction.
- Assist the Clinical Manager of DRI in educating staff and patients on new medications and diabetes management techniques.
- Maintain surveillance and ensure nursing staff competency in diabetes management within the hospital.
- Serve as a preceptor for new staff members.
What You'll Bring:
- BSN from an accredited School of Nursing required
- Bachelor's degree from an accredited college or university with a coordinated undergraduate program
- RN experience with at least 2 years of medical/surgical nursing preferred, with emphasis on diabetes patient education
- Registered dietitian experience with at least 2 years in clinical dietetics and diabetes patient education
- Current registration in the State of NH as a Registered Nurse
- Basic Life Support (BLS) certification required
- Current registration in the State of NH as a Licensed Dietitian (LD), maintaining registration with the Commission of Dietetic Registration of the American Dietetic Association
- Certified Diabetes Educator (CDE) certification preferred; if not certified, must obtain certification within two years of hire
