Social Work/Case Manager (Per Diem) - Case Management

Who You Are:

Catholic Medical Center is seeking a dedicated, compassionate social worker with a strong understanding of case management, healthcare quality practices, and are adept at collaborating with diverse healthcare teams to ensure patient-centered care. Your expertise includes outcomes research, managed care quality mandates, and the ability to handle complex discharge planning while maintaining confidentiality and respect for patient dignity. Your ability to manage diverse duties with sound judgment and attention to detail sets you apart.

What You'll Do:

  • Collaborate with nurses, physicians, and the healthcare team to support patient care and discharge planning.
  • Provide counseling, crisis intervention, and support for patients (neonatal, pediatrics, adolescent, adults and geriatrics) and families.
  • Monitor patient progress to ensure timely and appropriate care.
  • Develop and implement comprehensive discharge plans, facilitating community resource connections.
  • Coordinate family meetings, facilitate advance care planning, and provide education/support groups.
  • Serve as a liaison to community agencies for discharge planning and continuity of care.
  • Participate in daily rounds, interdisciplinary meetings, and policy development to improve patient care processes.
  • Conduct biopsychosocial assessments to evaluate patient/family readiness for discharge.
  • Assist with quality management and data collection to improve patient care and resource utilization.
  • Perform fit testing, facilitate PASRR screenings, and adhere to State DHHS requirements for nursing home placements.


What You'll Bring:

  • Master of Science in Social Work.
  • 1-3 years of healthcare system experience, preferably in a hospital setting.
  • Knowledge of outcomes research, physician profiling, managed care quality mandates, case management, principles and practices of healthcare quality management and TQM, statistics and utilization management.
  • Ability to plan and perform diversified duties requiring an extensive knowledge of a particular field and the use of a wide range of procedures. Involves the exercise of judgment in the analysis of facts and conditions regarding individual problems or transactions to determine what actions should be taken, within the limits of standard practice.
